| Child Medical Form | Download |
| Every child attending FAB Care must have a medical form and shot record on file. Forms must be renewed every two years. Shot records must be updated as new immunizations are given. |
| Medication Authorization Form | Download |
| Any child who requires medication while at the center must have parent authorization before the teacher can dispense the medicine. |
| Pick-up Authorization Form | Download |
| Every child needs a signed pick-up authorization from parent. It must include parent(s) and at least 2 others who can be used when the child needs to be taken home. Please include anyone you might call upon, even if it is rare. No one will be allowed to pick-up children without this signed permission. |
| Emergency Card | Download |
| Every child needs two signed and notarized emergency medical treatment authorization cards on file at child care. Parents are always notified when a medical or other emergency arises, however, these cards will help us get your child emergency treatment, until you can arrive, if necessary. |
| Parent contract | Download |
| Each parent must sign a contract stating what will be paid and how it will be paid each week. This contract also has a place for parents to authorize water play, sunscreen use, and use of child's picture for other than outside the classroom. |
